IF YOU HAVE KNOWN HEALTH ISSUES

Any YES answers on the Medical waiver will require sign off by a medical professional on a standard RSTC medical form. The form can be generic or branded from any training agency.  RSTC forms are valid for one calendar year. This is only required if you have indicated yes on the online medical waiver.

This form can preferably be scanned and sent to us electronically, but you can also bring a signed paper copy with you.
